Maddie's Spay/Neuter Project NYC
Low-cost spays and neuters for cats and dogs
for New York City residents with a valid Medicaid card
Having your cats and dogs spayed or neutered makes
good sense — for you and the animals.

their associated costs) later in your cat's or dog's life. |

Can help eliminate some problem
behaviors. |
If you reside within the five boroughs of New York
City and have a valid Medicaid card, you can have up to a total
of six cats or dogs (or a combination of cats/dogs totaling no more
than six) spayed or neutered each year for a special low price at
participating veterinarians and non-profit organizations.
Here's how it works:

Locate a participating veterinarian
or non-profit organization from the list provided below. |
| 2) |
Call the veterinarian or organization
and make an appointment for the surgery. |
| 3) |
Bring your Medicaid card and photo ID with
you when you take your cat(s) or dog(s) in for the appointment.
Your card will be photocopied at the appointment. |
| 4) |
The veterinarian will perform the surgery. |
| 5) |
Pay for the surgery: $10 for
each cat, $20 for each dog; up to a total of six animals within
a 12-month period. |
Note: These fees apply to the spay or neuter surgery
only. If your pet requires additional care, such as pre-surgery
testing of an older pet or other procedures, those costs will be
in addition to the above fees. You will be informed prior to the
surgery if additional costs will be incurred.

| About Maddie's Fund
Maddie's Fund, The Pet Rescue Foundation
(www.maddiesfund.org),
is a family foundation funded by Workday and PeopleSoft Founder
Dave Duffield and his wife, Cheryl. Maddie's Fund is helping
to create a no-kill nation where all healthy and treatable
shelter dogs and cats are guaranteed a loving home. To achieve
this goal, Maddie's Fund is investing its resources in building
community collaborations where animal welfare organizations
come together to develop successful models of lifesaving;
in veterinary colleges to help shelter medicine become part
of the veterinary curriculum; in private practice veterinarians
to encourage greater participation in the animal welfare cause;
and in the implementation of national strategies to collect
and report shelter statistics. Maddie's Fund is named after
the family's beloved Miniature Schnauzer who passed away in
1997.
Maddie's Spay/Neuter Project is partnering
with Maddie's Pet Rescue Project to end the killing of healthy
and treatable shelter pets in New York City within ten years.
